An employer action is a separate step
For DHA activation, the hiring facility applies and the professional approves the request. The facility must have the relevant active service and meet the activation prerequisites. For MOHAP licensing, the published service requires a valid evaluation and acceptance of the work invitation. Neither step is replaced by a passed exam.
Send the facility representative a concise status summary: intended title, relevant registration or evaluation, unresolved requests and the next action you understand is needed. Ask them to confirm the correct service and any facility-side prerequisite before relying on a start date.

Read the exact request and any deadline shown. Match each question to its evidence, correct the relevant field where permitted and keep the response record. A return for information should not be described as approval or assumed to be a final rejection.
